New Client FORMS

 New Clients need to fill out the  following Intake forms available in multiple formats.  Information may  be typed or written out before hand.  Or, you can come 20 minures early to your appointment to fill these out. These need to be filled out and  signed prior to your first visit.

• Client information sheet in MSWord 2003 and PDF. 
• Confidentiality Agreement in MSWord 2003 and PDF. 
• Payment, Office Policies, and Theraputic Contract Wordand PDF.
